Nomination Guidelines

New Prize—New Submissions Requested
Nomination Deadline Extended to March 29, 2024

 

In order to be eligible for the Joan H. Tisch Community Health Award, a nominee must be:

  • An organization or an individual in the New York City Metropolitan area (including the five boroughs, Long Island, and Westchester, Putnam and Rockland Counties.)
  • Involved in a community-based public health initiative in an urban setting that has had measurable impact.
  • A tax-exempt nonprofit under Section 501(c)(3) of the Internal Revenue Code if an organization; or affiliated with such an organization if an individual

 

Nominees are judged on the following criteria:

  • Achievement: Outstanding accomplishment and future potential in the development and implementation of a community-based public health initiative in an urban setting.
  • Imagination: Demonstrated originality, creativity and innovation in tackling an urban public health problem.
  • Impact: Positive and lasting improvement in health, well-being and community life for a significant proportion of the target population, and potential for replication in other communities.

 

Nominator/Reference Eligibility and Nomination Process:

  • Nominators and references are persons knowledgeable about the nominee’s contributions to improving urban public health.
  • Nominators and references may not be the nominee or a member of his or her family.
  • If the nominee is an organization, the nominator and references may not be an employee, officer, or any other individual receiving payment for their services from that organization, or a member of their family.
  • If the nominee is an individual, the nominator and references may not be an employee, officer or any other individual receiving payment for their services from the individual’s affiliated organization to which half of the prize monies would be payable, or a member of their family.
  • Board members (of an organization nominee or of an individual nominee’s affiliated organization) may nominate or serve as references if they do not receive payment for their service from the organization. However, of the three required letters — nominator’s narrative, reference 1 and reference 2 — only one may be from a board member.

Re-nominations:

If you nominated an individual or organization in the past three years, there is an expedited re-nomination process. You (the nominator) may simply send an email indicating that you would like the nominee to be considered again this year. However, that being said, we would invite (but not require) nominators to provide a 1–2 page update to the previous submission informing the Selection Committee of additional noteworthy achievements since the person or organization was last nominated. Also, please make any necessary changes to the contact and other information from the original nomination form. Later in the process, if the nominee should advance to the semifinalist or finalist stage, we may ask for a new letter of reference or other documentation which the Selection Committee deems necessary to update the application.

You may also elect to prepare a completely new application using the Nomination Guidelines and Form, including new letters of reference, but this is not mandatory. However, if you first submitted your nominee more than three years ago, you are required to prepare a new application using the Nomination Guidelines and Form, including new letters of reference.
